On the PIP form, the declaration, as I am filling this all in on my son Glen's behalf I would have to sign it?  and where is says PRINT YOUR NAME  do I print my name or Glen's?  I would appreciate your help.

Were you made appointee for Glen when he turned 16 ? If you are his appointee, then you sign the form

Yes I am Glen's appointee, I've also applied for deputyship.  Thanks for your help.
